- There is no point blaming an individual for bad processes, find the root cause behind the poor system instead
- System changes in procedure and responsibility must begin with management, they can not be made effectively from the ground floor up, it must be inspired from management downwards
- Working harder will not solve system failures, only be recognizing and addressing them can improvements start to be made
